NEW TO OUR OFFICE?

Whether it’s your first visit to our office or your fifth, we strive to make every experience feel like you’re part of our family. To help us provide the best care, please bring your insurance cards, photo ID, current glasses or contacts, and a list of medications to keep your records up to date.

If you’re new to our office, we’ve provided a link to our online health intake forms (click on the envelope below), which you can conveniently fill out prior to your exam.

Accepted Insurance Networks

While we’d love to see you regardless of your insurance network, we’ve provided a list of the insurance plans we can bill directly through our office. If you don’t see your network listed, please give us a call to confirm whether we are in-network for your plan.

For out-of-network plans, we’re happy to provide all necessary documentation to help you submit a reimbursement request. Let us know how we can assist!
